CMS has released the coding changes for January 2021 and the new and established patient codes for office/out-patient are changing, along with the required documentation. Below are some of the highlights:

  • CPT 99201 is being deleted.

  • There will be no required level of history for CPT 99202-99215. From the AMA: "The extent of history and physical examination is not an element in selection of office or other outpatient services."

  • The visit time will be defined as total time spent, including non face-to-face time, and will no longer strictly include time spent on the examination and counseling.

  • The Medical Decision Making (MDM) guidelines were revised and include the following three elements: number and complexity of problems, amount and/or complexity of data to be reviewed and analyzed and risk of complications and/or morbidity or mortality of patient management.
